Sisters of Synergy provides housing-focused support designed to help eligible participants navigate barriers, maintain stability and stay connected to the resources needed to move forward.

Our team helps participants navigate housing barriers, understand their responsibilities, connect with community resources and build a foundation for long-term housing stability.
How We Help
Housing With Supports operates through Sisters of Synergy Home Care Services, a DBA of Sisters of Synergy LLC. Services are provided according to applicable program, county, funding and eligibility requirements.
Depending on the program and participant's approved needs, housing-focused support may include:

Coordination and communication with property managers and other approved partners

Referrals to community resources and supportive services

How To Access Housing With Supports
Housing With Supports is currently a referral- and eligibility-based program, and enrollment depends on available program capacity.
Individuals may be referred through Coordinated Entry or another approved referral source. If you are not currently connected to a referral source, you may complete our Housing Interest Form. Our team will review your information, explain possible next steps, and let you know whether we currently have an available opening or whether another housing resource or referral pathway may be appropriate.
Submitting an interest form does not guarantee enrollment, housing placement, rental assistance, financial assistance or an available program opening.
Our Approach
Our goal is not simply to help someone obtain housing. We want housing to become part of a stronger foundation for long-term stability. We work with participants to identify barriers, understand responsibilities, connect to resources and develop practical next steps.
